Welsh social care platforms shortlisted for Accolades

Three innovative projects nominated for ‘Working in partnership’ award that recognises the work of social care teams across Wales 

The Accolades awards ceremony is due to be held on May 1, 2025. This year, 11 projects and seven individuals have been shortlisted for awards in six categories that recognise notable work in social care, childcare, play and early years across Wales. Three innovative online platforms are nominated for the ‘Working in partneship’ category.

This category is sponsored by Life Sciences Hub Wales, an organisation that aims to drive the development and adoption of life science innovation in health and social care in Wales. The three shortlisted entrants are:  

  • Connecting Carmarthenshire
    This partnership project run by Carmarthenshire County Council aims to help residents live and age well, working with third-sector organisations to ensure people receive the right services at the right time. The platform makes available a range of services from local providers, including suicide prevention, domestic abuse services and community engagement projects. Specialists from different organisations can also co-ordinate a person-centred approach to help the people they support achieve the best possible outcomes. 

  • Delta CONNECT
    This collaborative approach by local authorities, the local health board and community services across West Wales aims to improve well-being among older adults with chronic conditions and those at risk of social isolation. The platform combines social care and healthcare services, using technology and personalised support such as proactive calls and a 24/7 response service to provide preventative and proactive care. Using this service patients are discharged from hospital on average five days earlier than without it.

  • Wrexham County Borough Council’s Well-Being Portal 
    Launched in 2023, this portal aims to reduce barriers to receiving support, requiring users to share their information just once, rather than with each time they speak to a different local authority, health or third-sector organisation. It was created in response to feedback from those who had experiences issues in provision and provides information for young people, housing and tenancies, and support for families. 

Cllr John Pritchard, Lead Member for Adult Social Care at Wrexham Country Borogh Council, says: ‘The portal just makes life so much easier for people trying to access services, and as a council we want to make it as easy as possible for families and individuals to get the support they need.’ 

Cllr Rob Walsh, Lead Member for Children’s Social Services, adds: ‘Lots of families and parents use the portal to get help and advice on a wide range of topics, and it’s proved to be a really useful tool. It’s a great example of how online services can be used by social care providers to support people, and we’re delighted that the portal has been shortlisted for an Accolades award.’
